Things to Remember/District Nursing Policies

• The Student Emergency Card MUST be completed online or in paper form and returned to the nurse’s office as soon as possible. A new card must be completed for each child at the beginning of each school year in order for the school nurse to provide treatment and/or medications. Please remember to keep the nurse's office updated on any changes that may occur during the school year. Please keep in mind, we are able to give certain over-the-counter medications, such as Tylenol (acetaminophen) or Motrin/Advil (ibuprofen) IF the parent/guardian has completed and returned the Emergency Treatment Data Card and checked yes. Please be sure to fill out the Emergency Treatment Data Card in its ENTIRETY and sign in the designated three areas. The Emergency Data Card can be downloaded and printed out DIRECTLY from the Christina School District website. For your convenience, I will attach the links, in both English and in Spanish, to the bottom of this page. These will direct you to the 2021-2022 Student Emergency Card.
• For their safety, PLEASE NOTIFY THE SCHOOL NURSE IF YOUR STUDENT HAS A CHRONIC CONDITION! (asthma, allergies, diabetes, seizure disorder, etc.) A specific action plan needs to be on file in order to properly care for your student.
• When sending in any medications, whether prescription or over-the-counter, a written permission form must be completed. All medication must be in a labeled original bottle. Please note that any medication classified as a controlled substance must be transported to the school by an adult.   • It is IMPERATIVE that we have a rescue inhaler for ALL students with the diagnosis of asthma. The inhaler can be kept in the nurse’s office or the student may carry the inhaler with them during school hours with signed consent from the parent or guardian.
• 7th-grade health screenings will be completed during the Fall and consist of hearing, vision, and orthopedic screenings. 6th and 8th grade will have an orthopedic screening completed during the school year. If you have any questions regarding screenings, please feel free to contact the school nurse.
• All students that are new to the Delaware public school system must have a physical exam, completed within the past two years. Students will receive written notification to have the physical completed within 14 days or have a note from their doctor stating an upcoming doctor’s appointment. • In order for any student to participate in a Delaware Interscholastic Athletic Association (DIAA) sport, they must have a yearly physical completed. These forms must be completed after April 1st each year based on a physical performed by the signing physician within one year of the date of signature. • Please ensure that we have an accurate, up-to-date record of your student’s immunizations. Students with incomplete immunizations will receive written notification to have immunizations completed within 14 days of notice. Immunizations required for 6th, 7th, and 8th graders are as follows:
• 5 doses of DTaP/DTP Td (unless 4^th dose was given after the 4^th birthday)
• 4 doses of IPV or OPV (unless 3^rd dose was given after the 4^th birthday)
• 3 doses of Hepatitis B
• 2 doses of MMR
• 2 doses of Varicella or written disease history by a licensed healthcare provider
• Please also consider talking to your student's physician about the following vaccines:
• Influenza
• COVID-19 (the CDC recommends everyone 12 years and older get vaccinated)
• Meningococcal (1 dose will be required for 8^th graders prior to entering 9^th grade)
• Tdap (booster) (1 dose will be required for 8^th graders prior to entering 9^th grade)

• Breakfast is served here at Gauger-Cobbs every morning at no charge. Breakfast starts at 7:00 a.m. and ends at the beginning of homeroom. School breakfast meets USDA requirements. Students should take advantage of this service as it provides them with a fourth of their daily requirements and studies show that students who eat breakfast daily do better in school!
