Welcome to the Brennen School Nurse's website! Please browse through the information and forms provided below. If you have any questions or concerns, please feel free to call or email us. Our first concern for your child is to provide a healthy and safe environment for learning. Besides running the school health office where children are seen for injuries and illness, the school nurse also screens students annually for vision and hearing, as well as scoliosis screenings for those students in grades 5 through 9. Parents are encouraged to communicate directly with the nurse regarding any health concern that may impact the student’s day in school. The nurse cannot make a medical diagnosis or prescribe medical treatment and cannot administer medications without written physician’s orders. Injuries that occur at home must be taken care of at home. A student should not come to school for the nurse to determine if s/he is well enough to attend. If there is doubt regarding a student's health, the student should remain home and your family physician should be consulted.
To ensure the best possible care for your student, please be sure to return an updated Emergency Card at the start of each school year. Please alert the nurse if there is a change in your child’s health status so that your child's emergency medical information can be updated. Likewise, any change in your address and home, work, or cell phone numbers should be brought to our attention immediately.

Nemours is making it easier for school nurses who work in Delaware public schools to be a part of your child’s care team. They can log on to NemoursLink® , electronic medical records system, with their computer. There, they can see a child’s plan of care and information about almost every visit to Nemours/Alfred I. duPont Hospital for Children or a Nemours Pediatrics office. School nurses can only view a child’s records if a parent or guardian has signed a form in advance. Delaware school districts signed up to participate so that school nurses can connect to Nemours information about students at their school. The program that makes this possible is called The Student Health Collaboration.

To be completed by licensed healthcare provider:
Physician (MD or DO), Clinical Nurse Specialist (APN), Advanced Practice Nurse (APN), or Physician’s Assistant (PA)
To Parent or Guardian:
In order to provide the best educational experience, school personnel must understand your child’s health needs. This form requests information from you (Part I) and your health care provider (Parts I, II, and III). All students in Delaware public schools must provide documentation of current immunizations, and a current (within 2 years) physical examination upon school entry and at ninth (9th) grade.
